In some instances, personal information may be obtained from referring doctors, treating doctors, fertility specialists, partners, or family members when it is not practicable to obtain it directly from the individual. However, any information obtained from third parties will always be confirmed with the individual when it becomes practicable to do so.
  2. Purpose of Collection and Use Personal and health information is collected and used for the following purposes:
    • To provide individuals with information about OFC’s services
    • To ensure the provision of the best possible care to patients of OFC
    • To manage the health services effectively
    • To send communications (including results) to individuals and treating doctors
    • To provide information, advice, and conduct business processing functions
    • To update records and keep contact details up to date
    • To respond to any complaints made by individuals
    • To comply with laws, regulations, determinations, and directions of regulators or governmental authorities
    • For internal administrative, marketing, planning, product or service development, quality control, and research purposes of OFC and its related bodies corporate
  3. Accuracy and Completeness of Information It is important to provide accurate and complete personal and health information. If any information provided is inaccurate, incomplete, or withheld, it may affect the services OFC can provide, and in some cases, OFC may be unable to provide services at all.

Information is stored in both electronic and hard copy formats and is destroyed or de-identified when no longer needed, following the relevant legislative requirements.
